The new patient form is an aid in gathering information for the initial evaluation. Please download and fill out the form prior to your first appointment. If you have difficulty filling out parts of it or cannot fill it out at all, that is okay. Just come to the initial session and we will go over most of these subjects in person in the initial evaluation.
